Specimen Receipt & Rejection
- All specimens must be accompanied with a completed requisition
- The requisition must clearly state the patient's name, DOB, requesting physician, and test being ordered.
- All specimens must be clearly identified with the patient's name or the numbered identification sticker from the front of the requisition. Improperly labeled specimens will be handled as problem specimens and the submitting nurse or physician will be notified.
- All specimens must be received in good condition, submitted in secondary bio-hazard transport bag and in the appropriate fixative. In some cases, refrigeration may be accepted in place of fixation.
